Social Prescribing Link Workers / Health and Wellbeing Coach -Northumbria PCN
Our Social Prescribing team all have varied backgrounds and lots of experience in different sectors that allow them to deliver a wide knowledge of support to our patients. We cover all eight surgeries within the Valens Partnership to offer the same service to everyone. .This is a person led service where we listen to the patient to really find out what matters to them. We are empathetic and non judgemental and welcome all enquiries and referrals from patients, our partners or external organisations.
We are currently a team of three and will be expanding to four with the appointment of a Health and Wellbeing Coach soon.
Who can use this service?
Patients can self refer into our service using email or telephone.
How are services delivered?
We offer face to face appointments and also telephone appointments. We can also offer home visits if a patient is unable to leave their home.
