Social Prescribing Link Workers / Health and Wellbeing Coach -Northumbria PCN

Our Social Prescribers and Health and Wellbeing Coach all have varied backgrounds and lots of experience in different sectors that allow them to deliver a wide knowledge of support to our patients. We cover Cramlington, Seaton Valley, Morpeth, Lynemouth and Pegswood.This is a person led service where we listen to the patient to really find out what matters to them. We are empathetic and non judgemental and welcome all enquiries and referrals from patients, our partners or external organisations.

Who can use this service?

Patients can self refer into our service using email or telephone.

How are services delivered?

We offer face to face appointments and also telephone appointments. We can also offer home visits if a patient is unable to leave their home.
