Our charity provides commissioned Day Services (social care) for 160 adults with learning disabilities. We support conditions such as Down's, Cerebral palsy, Prader- Willi and Smith Magenis Syndromes, and also autism. 60% of our clients have co-morbid physical or sensory disabilities. We have four day centres in the region, three of which take Northumberland referrals. Clients can either be referred in by Adult Social Care or the NHS as part of a Day Service contract (their Care Support Plan or Continuing Healthcare Plan), or can self-refer.
- We provide a timetable of daily activities which enable adults in our care to learn life skills, work skills and to manage their health & disability. All activities are undertaken in groups of 8 - 10 adults both at our Day Centres and, working with our specialist Learning Disability Support Workers, Volunteering Lead and Work Coaches, off site. Our aim is for adults with complex learning disability to have the skills and confidence to lead active, inclusive and fulfilling lives in their own community.
- We have four Day Centres (Hubs): Acomb and Blyth in Northumberland, Kenton in Newcastle and Coundon in County Durham. Each Hub not only provides activities for Clients in Day Service but works with Clients to provide vital services to support the local community. These include meals services for older frail adults living locally, friendship groups for local age-frail adults (hosted at our Day Centres by Staff and Clients), community cafes/kitchens, a 'pay what you can' food and second-hand uniform shop, horticulture and woodwork services. These enable our Clients to learn work skills, but also to integrate in the community, being visible and valued for their service.
- We work on a Person-Centered Positive Behaviour Support basis, all programming shaped to meet Clients' Care Support Plan outcomes and aspirations. Each Client will have a Key Worker at Journey who will attend annual reviews and work closely with the Client throughout their journey with us.

Adults with learning disability aged 18+. We also support adults with co-morbid autism.
Face to face at our Day Centres (Hubs) in small groups of 8-10 adults:Open : Monday - Friday 9.30am - 3.30pm, and also in the community.
