HDFT - Growing Healthy 0-19(25) Northumberland
The Growing Healthy Northumberland 0-19(25) Service is for children, young people and families in Northumberland. Our 0-19(25) team provide a wide-ranging service to children and their families to support health and wellbeing. We have a 'Children's Health Service-HDFT' App which offers advice for children, young people and families in Northumberland for the journey from antenatal care into adulthood. We work in partnership with other agencies to ensure children and young people grow up to be healthy, make the right choices about their health and to safeguard their welfare.
The 0-5 team (Health Visiting Team) works with parents and carers to ensure that children have the best start in life from pregancy through to early years to ensure children achieve to their full potential and are ready to start their school journey.
The 5-19 Team (sometimes known as School Nurses) promote and protect the physical health and emotional wellbeing of children and young people who are school age.

To refer into our 5-19 service for any child or young person in need of additional support, please use the Multi-Agency Referral Form(MARF). https://form.northumberland.gov.uk/form/auto/multi_agency_ref_form
For those children under the age of 5 they will have an allocated Health Visitor. Please contact SPOC Tel:0300 373 2488 and they will direct your call to the most relevant member of the 0-5 team to discuss your query. (Or email : hdft.spocnorthumberland@nhs.net)
Our team can also be contacted via the Health Chat feature on our 'Children's Health Service-HDFT' App which gives access to a same day response for a 0-19 team member, Monday to Friday 09:00-17:00.

Families of Children aged 0-19 or 25 with SEND living within the Northumberland area
