NCC: Housing - Healthcare Navigation

The new and exciting role of Healthcare Navigator has been created to support all Northumberland residents with complex, multiple needs and from inclusion groups such as refugees and asylum seekers, ex-prisoners, people experiencing homelessness and addiction to access healthcare and other care services across Northumberland. We will have a vital role in sharing the experiences of our residents with services and commissioners to tackle inequalities and improve health outcomes for these groups.

We are a newly established service sitting within Northumberland County Council Housing funded by the NHS North East & North Cumbria Integrated Care Board.

Who can use this service?

How are services delivered?

All Northumberland residents experiencing inequalities in accessing healthcare. 

Ideally telephone, email or virtually if circumstances allow. Home visit or community setting meetings can also be arranged if needed.
